HSE Blind Spots by ROC Advice
Insight into work as planned and work as done.
An engagement tool that shows what your leaders, supervisors and crews really think about your critical risks. The insights give you clear, targeted focus areas to go to the field and prove.
Traceability
Follow each critical risk from the plan, procedure and control through to what people see on shift, so you know where the line holds and where it breaks.
Engagement
Leaders, supervisors and crew, including contractors, answer the same questions. Everyone's view counts, and the people doing the work are heard.
Insights
Clear, targeted focus areas: where people believe controls hold, where they don't, and where to go and prove it in the field.
Work as planned versus work as done
Plans, procedures and controls are written before the work starts. The work itself happens under time pressure, with whatever is on the day. The two drift apart, and nobody planned it that way.
Research backs it up: managers consistently rate safety more positively than the people doing the work. That gap isn't a blame issue. It's often where things go wrong, and it's hard to see from either side on its own.
HSE Blind Spots puts both views side by side, so leaders and crews can agree what's really happening and what matters most.
You can't be everywhere at once. Field conversations happen with whoever is there that day. Night shift, shift handovers and new starters are easy to miss, and one-off conversations aren't measured or tracked. HSE Blind Spots asks everyone the same questions, then breaks the results down by shift, crew and time on site, so those groups are measured and tracked over time.
Then go and prove it. The survey tells you what people think, not whether a control works. That's the point: it gives leaders and verifiers a short, targeted list of focus areas to prove in the field, instead of checking everything equally.
What it gives leaders
Reach every shift
Night shift, shift handovers and new starters, not just whoever is on site when you visit.
Timing
Run it before a shutdown, a campaign or a change, so you act while there's still time to fix things.
Measuring
The same questions each time, so you can see whether a gap is closing or growing.
Changing currents
Pick up shifts in how people see the work early, before they become bigger problems.
Pick the module that matches your biggest risk
Each module is 10 to 12 questions, built for energy and resources. Most clients start with one; larger runs add a second.
Critical controls
Do people know the fatality risks and their controls, and are those controls proved in the field? Aligned with ICMM critical control management and IOGP guidance.
Shutdown readiness
Run 3 to 6 months before a shutdown or turnaround: scope, permits and isolations at peak, contractors, fatigue, and lessons from the last one.
Digital HSE
Where the HSE admin load sits and what hurts most, from permits and JSAs to double handling and hand-built reports. Shows where digital tools would save the most time.
How Digital HSE worksAlso available
ISO 45001 readiness
How your OHS management system works in practice, including the 2024 climate amendment, and what to prepare for the 2027 revision.
ISO 14001:2026 readiness
The new edition's changes: biodiversity and resources, life cycle thinking, change management and outside providers, before your next certification audit.
Contractor management
Whether contractors are chosen, briefed and supervised on HSE capability, and whether they're part of the same safety system.
HSE system basics
For juniors and explorers building a first system: the main hazards, simple procedures, inductions, reporting and emergency arrangements.
Psychosocial risk
A short screen of workload, rosters, support, change, remote work and harassment, aligned with Safe Work Australia's Code of Practice, with People at Work for the full assessment.
Something else?
If your biggest risk isn't listed, we can build a module for it on the same template.
How it works
About four weeks from start to report, with 10 minutes per person.
- 01
Agree
A short call to pick the module, the groups and the sites, and to agree how results are shared.
- 02
Survey
Leaders, supervisors and crew, including contractors, answer by QR code at pre-starts or by link. Answers are anonymous.
- 03
Report
Scores for each group, the gaps between leaders and the field, and what the crew said, grouped into themes.
- 04
Consultant review
Optional. Two hours with your leadership team on the top three blind spots, with written recommendations.
What you get
One index score
A single HSE Blind Spots score out of 100, so you can track change from one survey to the next.
A visual report
Six to eight pages: your maturity level, leaders versus crew, a heat map and the top three blind spots.
An online dashboard
Optional, by annual subscription. Filter by role, site, crew or contractor. Groups under five people are never shown.
A consultant review
Optional. A senior HSE consultant takes your leaders through the results and recommends what to fix first.
Also included
- No limit on the number of people who take part
- Your logo and colours on the survey and the report
- A kick-off call, and a call to walk you through the report
- Results by role, site, crew, shift, employer and time on site, including contractors and new starters
- Your data in Excel and PDF, with comments grouped and names removed

Why HSE Blind Spots
General safety culture surveys measure how people feel about safety. HSE Blind Spots is built for energy and resources, and asks about the risks that can kill people.
| General safety culture surveys | HSE Blind Spots |
|---|---|
| General questions about culture, wellbeing and engagement | Modules on your critical risks: critical controls, shutdown readiness, HSE admin, ISO 45001 |
| One score for the organisation | Leaders and the crew scored side by side, so the gap between work as planned and work as done is the finding |
| Results interpreted by survey and culture specialists | An optional review by a senior HSE consultant who has worked shutdowns, drilling and operations in energy and resources |
| A culture score and themes to act on | The top three things to fix, and practical HSE help to fix them if you want it |
| Scored on a general culture model | Scored on a five-level maturity scale built around critical controls |
Five maturity levels
Your results place you on one of five levels, for leaders and for the crew separately. The gap between the two is the blind spot.
- 01
Basic
Controls depend on individuals. Little is written down or checked.
- 02
Simple system
A system exists, but how it's used in the field is patchy.
- 03
Critical controls
Critical risks and their controls are known and mostly checked.
- 04
Proven in the field
Controls are verified at the workface, and failures are acted on.
- 05
Benchmarked
Performance is tested against peers and keeps improving.
Levers that close the gap
Officers have a legal duty to verify that the processes managing risk are actually provided and used, not just to receive a report that they are. A blind spot shows where "it's done" and "it's working" have come apart. Each one points to a lever leaders can pull.
Leaders in the field
A planned routine for leaders to see the top critical controls working, on the shifts and tasks where the risk sits.
Clear standards for "working"
Each critical control described in terms a person can prove at the workface, not just tick on a form.
Backing for stop-work
How stops are handled, shared and treated in contractor KPIs, so stopping feels safe for everyone.
Less admin, more field time
Paperwork moved off supervisors' desks so they can be with their crews when it matters.
Contractors in the loop
Changes to the job or plan reach contractor crews before work starts, not after.
Independent verification
Field checks of critical controls by someone with no stake in the answer, aimed at the gaps the survey found.
Run the survey again 6 to 12 months later to show whether the gap has closed.
Built on research
Every module is aligned with a recognised framework, and we say exactly what each part is and isn't.
| Part | Based on |
|---|---|
| Critical controls and shutdown modules | ICMM critical control management, IOGP barrier and Life-Saving Rules guidance, ISO 45001 |
| Standards modules | ISO 45001:2018 (with the 2024 climate amendment) and the 2027 revision; ISO 14001:2026 |
| Leaders versus the field | Published research showing managers rate safety more positively than workers (Chen et al., Journal of Safety Research, 2010) |
| Psychosocial risk | Safe Work Australia's model Code of Practice, with full assessments using People at Work, the free validated tool from Australian WHS regulators |
| Benchmarks | Built from our own anonymised results over time. Until there's enough data, you're scored against our maturity scale and your own last survey |
